National Social Prescribing Day 2025 Event

Last week on Wednesday 19 March it was National Social Prescribing Day and our Social Prescribers attended an event at the Harrison's Hub Community Corner!

It was a great opportunity for the Social Prescribers to share good practice tips, peer support and identify ways that Social Prescribers can work cohesively as the Derby City Social Prescribing team.

Our Social Prescribing team supports patients from Derby North PCN & Derby South PCN GP surgeries who are experiencing social needs, such as housing issues, financial hardship, employment difficulties, loneliness isolation and emotional welfare.

Their role is to link patients to community groups and services to provide short and long-term input, with the view of improving a person’s health and wellbeing.
