Home From Hospital Volunteer

Are you able to support someone safely home from the discharge team at hospital and back to their home?

As a Home from Hospital Volunteer, you will have the opportunity to connect with a member of your local community who has been discharged from hospital and could benefit from your support.

Often, people can be discharged from hospital but have no one to help them get home, or carry out simple but often time-consuming tasks, or check that everything at home is OK for them for when they return and that they have everything they need. It could be as simple as a phone call to check on how they are doing or helping them with practical support such as food shopping, changing a light bulb, or helping to co-ordinate tasks to help the home be ready for their arrival.

It could be a relationship that only needs to last a short time until the person feels better or a longer term connection. The amount of involvement will depend on how much you wish to have and the needs of the person you are supporting.
