Welcome to Derby LINk.

At Derby LINk, we are always looking for ways to improve our engagement with the public and service providers as well as increase members' enjoyment of and participation with the project.  In order to help us provide you with more relevant information and opportunities to get involved, we would like to invite you to take part in this survey.

The aim of the survey is to identify the areas of health and social care that you are most interested in or would like to find out more about.  It shouldn't take more than five minutes to complete, and every submitted survey will be entered into a prize draw to win a £25 Westfield shopping voucher.
